2/11/2023 EnchantaVaughn Approved 1. Behavior: Upon arrival, client opened the door for QMHP and excused himself from watching tv so that he could prepare for session. Client appeared to be in good spirit and happy as evidenced by him smiling and being eager to start the session with his counselor. The clientâs father appeared calm and engaged in full conversation with client about his day and the current emotions he was feeling. Mother reports client continues to fight his younger siblings. 2. Behavior: Client displayed great attitude in therapy setting and was praised for continuing to make progress with his daily life functioning. Client was seen displaying active listening skills, while following along with his assignment. Client was witnessed adhering to the rules and regulations in the home. Throughout the session the family was calm and engaging with one another. 1. Intervention: QMHP had one on one with client. QMHP processed clientâs thoughts and feelings. QMHP inquired about evening routine and previous activities. QMHP encouraged client to utilize positive self-talk as a means of decreasing anxiety and managing frustration when encountering difficult or challenging circumstances. QMHP talked to client about his confidence levels and how they can affect his day and self-esteem. QMHP praised mother for showing more consistency and efforts with assisting her son in understanding how to follow rules and increase his social skills. QMHP rewarded client with an incentive for his commitment to therapy and great work in the home. QMHP monitored client individualized behavior modification through collaboration with dad and grandmother. QMHP educated and practiced with the client on using breathing techniques and other anger management skills to assist him with maintaining a calm spirit. QMHP assisted client in monitoring his own behaviors in efforts to promote exercising self-control in focused areas such as respecting boundaries with others. QMHP modeled proper positive behavioral expectations for the client to display in the home. 2. Intervention: QMHP discussed with client the importance of conducting himself appropriately at all times. QMHP offered active listening skills. QMHP probed client to not be afraid and ask questions if he doesnât understand any of the in-home assignments so that he will not have the same failures. QMHP isolated client from distractions to decrease disruptive behaviors and to reduce excessive talking. QMHP guided client by asking him why he believed his family was important to him and what is the overall image that he wants to depict when it comes to his family. QMHP assisted client in creating goals for his family and understanding what role would be in achieving them. QMHP role played with client and demonstrated for positive behaviors and responses to increase positive interactions between him and the family. QMHP applauded the behavior of client and recognized how him and his father had been interacting positively. QMHP reviewed an article with the two that discussed relationship skill building and ways to better a relationship with one another. 1. Response: During initial conversation, father expresses how proud he was of client and how far he has come since the beginning sessions. Client agreed to behave at home more often. Client communicated a positive attitude when reviewing problem solving and self-control strategies. Client promised he would try his best to correct himself when faced with conflict. Client learned to avoid negative and defiant behavior and comply to rules. Client was seen following directions. Client and his father reported that they understood the importance of working towards his goals and objectives daily at home. 2. Response: Client participated in rapport building activity with QMHP. Client ensures heâs doing everything possible to protect himself, dad, and grandmother. Client appreciates being well taken care of and agrees that his living situation has changed drastically since leaving his momâs. Client admits that going in and out of hotels were starting to take a toll on his mental health and now that he has his own room, he feels completely safe and happy. Client shares that his dad and grandmother have gone out of their way to make sure he has everything he need, including clothes, shoes, haircut, food, electronics, a computer for school, and the love he requires to be successful and feel important.
